- Use the schema as your shopping list, create a Gravity Form with these fields, in this order, with these types.
- Once the form exists, swap the
id="form-id"in the shortcode for your real form id. - Bulk-import the CSV via Forms → Import (shipped in 4.1.22, see the release post).
